GP Referral Scheme

Crewkerne Aqua Centre operates a GP Referral Scheme linked with the local health authority and South Somerset District Council. We offer one to one inductions for clients and take into account medical conditions and injuries, designing and organising a suitable exercise programme. Pro-Active clients may be referred to use the gym or the swimming pool.

 
All clients are referred from their doctor initially to the GP Referral Health Promotions Officer who works for the local health authority, from here details are passed to the Active Lifestyles Officer and GP Referral Scheme Co-ordinator at South Somerset District Council who contacts the clients about the scheme. Clients are then given a choice of leisure centres to attend, Crewkerne Aqua Centre being one of the choices. The paperwork is then passed onto the centre and appointments can be made.  

Our experienced instructors will then set up an initial meeting where paperwork will be looked at, an assessment form completed and a discussion about what the client's aims and goals are regarding fitness and getting active. Clients will then make a second appointment booking, where they will receive a one to one induction and be shown how to use the exercise equipment and the exercises which are aimed to benefit them. These exercise can be either gym based or pool based.

The programme runs for 12 weeks with the aim of encouraging that person to stay active and to keep exercising after the 12 week course finishes, benefiting their lifestyle and increasing their activity levels.
